At Southwest Health Center’s Nursing Home, we approach everything we do with a focus on our residents. From choosing what time to start or end their day or choosing when to enjoy their mid-day meal or choosing what to eat, we respect and abide by our residents' personal choices.
Each individual deserves our utmost attention, respect, and affection and can expect to receive our best care. Southwest Health Center’s Nursing Home is a coordinated system of professionals who care for our residents on a very personal basis but ultimately, the resident is empowered to have a voice in many aspects of their daily lives.
Our nursing home, located at 808 South Washington Street in Cuba City, offers residents the choice of private or semi-private rooms that are well-maintained and attractively decorated. Residents are welcome to bring personal items, such as a rocking chair, pictures, a radio, or TV. And our living and dining areas are just as comfortable.

Resident-centered care. A staff that is second to none. Activities and entertainment directed by a Resident Council. These are a few of the reasons that Southwest Health Center's Nursing Home has been awarded five stars by the Centers for Medicare and Medicaid Services (CMS).
CMS has introduced a new rating system for the nation's nursing homes. The goal was to make it easier for consumers to make informed decisions when reviewing nursing homes. The concept is borrowed from the hotel industry where the star-rating system has become popular. A five star rating is considered the best of the best.
"This ranking system makes the evaluation of nursing homes less complex for consumers", noted Dr. Kevin Carr who serves as the Medical Director for Southwest Health Center's Nursing Home. "The ranking is based on three critical areas that directly impact patient care. I am very proud, but not surprised, that Southwest Health Center earned the best possible ranking."
CMS rates nursing homes based on specific criteria including surveys from the most recent three-year period in addition to staffing and quality measures. Performance based on those three categories led to a star ranking. A rating of one star is "Much Below Average" and a rating of five stars, the highest level, is "Much Above Average". Only 12% of the nearly 16,000 US nursing homes received five stars. Southwest Health Center Nursing Home is the only nursing home in Grant County to earn an overall five star ranking. Staff that is second to none.
Continuously trained in the latest techniques to care for our residents, the staff at Southwest Health Center Nursing Home is dedicated, professional, compassionate, and caring. The multi-disciplinary team of health care providers meets regularly to provide a consistent, up-to-date care plan for each resident. In addition to 24/7 access to physicians, there is a registered nurse in-house at all times.
Comprehensive rehabilitation.
Whenever possible, our goal is to return residents to their homes and their lifestyles. Through rehabilitation—such as after a stroke, surgery, or a fractured bone—many residents learn to function on their own. For others, our staff works with residents in the Restorative/Rehabilitation program to offer greater independence. On-site therapies include:
- Post-Acute Rehabilitation
- Occupational Therapy
- Physical Therapy
- Speech Therapy
Recreation and activities to delight.
In order to meet the unique interests of our residents, we offer diverse social and recreational activities. Our highly anticipated Mother & Daughter Banquet and Holiday Party are annual hits with residents and their families. Area clergy hold regular religious services on site. There is also an active Resident Council who meets monthly to discuss specific issues and make suggestions.
Delicious meals cooked with care.
Inspired by restaurant fine dining experiences, our dining rooms are tastefully decorated with fine china and flatware on linen tablecloths. Under the direction of our registered dietitian, three home-cooked meals are prepared on-site and served in our dining rooms . . . or in the resident’s own room, if he/she prefers. While some residents may require special diets as prescribed by his/her doctor, we provide many choices at meal-time along with snacks in between. Family and guests are always welcome to dine, too.

Southwest Health Center is honored to have served so many special people over the years. Many of our friends and family members have chosen to leave a lasting memorial erected on our grounds. Statues, benches and fountains are just a few of the memorials that we proudly display.
